摘要
目的探讨参脉注射液(SMI)对梗死心肌的保护作用并初步揭示其可能的机制。方法通过结扎犬冠状动脉左前降支复制急性心肌梗死模型,从急性心肌梗死(MIS)、心肌缺血程度、缺血范围、心肌三酶,即血清肌酸磷酸激酶(CK)、天门冬氨酸氨基转移酶(AST)和乳酸脱氢酶(LDH)的活性及机体的血流动力学等方面来探讨参脉注射液对犬急性心肌梗死的保护作用。结果SMI对急性心肌梗死犬能明显减轻心肌缺血程度,缩小缺血范围及MIS,降低血清CPK、LDH及AST活性,能明显减少犬急性心肌梗死时的血压降低,并明显增加左室内最大变化速率(±dp/dtmax)、心输出量(CO)及CI,说明可改善心肌舒缩功能,有助于对抗梗死后泵衰竭,对急性心肌梗死犬能明显增加心肌血流量(MBF),降低CVR,降低LVEDP升高幅度,同时延长舒张期,有利于心内膜下心肌供血。结论SMI对犬心肌缺血具有明显的保护作用,其机制可能是通过改善血流动力学,平衡心肌供氧与需氧,改善心肌代谢,保护受损心肌细胞等多种途径实现的。
Objective To investigate the protective effect and mechanisms of SMI on the myocardial infarction in dogs. Methods The model of the myocardial infarction in dogs was developed by ligating the left coronary anterior descending artery and the area of myocardial infarction, the content of LDH, CK, AST and homodynamic were detected. Results SMI reduced the degree of myocardial infarction and the myocardial infarction area, decreased the release of myocardial enzyme, resisted the decrease of SBP, DBP, LVP and + dp/dtmax and increase of LVEDP. Conclusion SMI has the protective effect of the myocardial infarction, and it may attribute to the ameliorating hemodynamic, decreasing cardiac oxygen consumption and improving cardiac metabolism.
