提高供肝灌注切取质量减少肝移植手术后胆道损伤并发症 被引量:4

Decrease the biliary complications after liver transplantation by improving the standard of procured and prepared technique

摘要 [目的]探讨肝移植术供肝的灌注、切取和修整方法,以进一步减少肝移植手术后胆道损伤并发症。[方法]总结分析2002年5月-2005年5月采用不同方法快速原位灌注、快速切取以及修剪的437例供肝。[结果]437例供肝无1例原发肝脏无功能发生,但67例患者发生轻重不同的胆道损伤并发症,其中3例患者行二次肝移植治愈,其他患者行胆道引流和胆道镜治疗治愈或好转。[结论]合理的供肝灌注、切取和修剪,可有效的减少肝移植手术后胆道损伤并发症。 [Objective] To explore the experience of procured and prepared technique of transplanted liver to decrease the b'diary complications after liver transplantation. [Methods] A total of 437 cases of transplanted liver prepared by different way for liver transplantation during May 2002 to May 2005 were retrospectively analysed. [Results] There was no case of primary nonfunetion. 67 cases suffered from biliary complication and 3 patients were cured by liver retransplantation, the others recovered by hiliary drainage or choledochoscopic cast extraction. [Camclusion] Rational procured and prepared technique are the key factor to reduce the biliary complications after liver transplantation.
