眼球筋膜下麻醉在眼科手术中的应用

Anaesthesia at infrafascia bulbi used in ophthalmic operation

目的探讨眼球筋膜下麻醉在眼科手术中的应用效果、优点及作用机制。方法由鼻上方近角膜缘球结膜下小切口伸入钝性针头并注射2%利多卡因和0.75%布比卡因1∶1混合液2mL,使局部球筋膜下充满药液,开展各种手术操作219眼。结果219眼麻醉效果满意,手术顺利完成,无麻醉并发症发生。结论球筋膜下麻醉避免了对视神经、动、静脉血管、眼肌及眼球壁的机械性损伤,是一种安全、有效、并发症少的麻醉方法。

Objective To study the advantage, indications and mechanism of infrafascia bulbi anaesthetic technique used in ophthalmic operation, Methods 2 ml of mixture of 2% lidocaine and 0.75% bupivacaine was injected at superonasal infrafascia bulbi through the blunt pinhead, which entered the fascia bulbi via a small incision and reached the equator of the eyeball, When the injection of the mixture filled up the fascia bulbi the surgery could be performed. 219 eyes was operated in this way. Results lnfrafascia bulbi anaesthetsla applied in 219 eyes and shown the good effect, The operations were underwent successfully without complication of anesthesia. Conclusions It could avoid the mechanical injure to optic nerve, artery, vein, muscle and wall of eyeball with infrafascia bulbi anaesthesia. It can be used safely and effectively for ophthalmic operation with less complication.
