超声引导经皮穿刺活检对乳腺占位性病变的诊断价值 被引量:44

摘要 目的探讨超声引导下经皮穿刺活检乳腺占位性病变的诊断和鉴别诊断价值。方法用16G及18G活检针于超声引导下经皮穿刺140例乳腺占位性病变,所取组织以能作出明确组织病理诊断为取材满意,否则为不满意。全部病例以超声引导穿刺活检或术后病理诊断为最后诊断。结果16G针活106例,18G针活检34例,每例取材2~3次,全部进行组织学检查。16G和18G针取材满意率分别为100%与97.1%。所有病例穿刺活检后均无严重并发症。超声引导下穿刺活检诊断乳腺占位性病变的灵敏度为98.0%,正确诊断指数为0.98,而超声诊断相应指标分别为83.7%和0.79。结论超声引导下经皮穿刺活检乳腺占位性病变的准确性高,并发症发生率低,是临床确诊乳腺占位性病变的有效方法。 Objective To investigate the value of ultrasound-guided core-needle biopsy (US-CNB) in diagnosis of space-occupying breast lesions (SOBL). Methods One hundred and forty SOBL patients received US-CNB using coarse needle (16G or 18G). Satisfied sampling was defined as the acquired tissue being large enough to give a definite diagnosis, otherwise as dissatisfied sampling. The pathology was taken as the final diagnosis. Results The number of patients receiving 16G and 18G were 106 and 34, respectively. The mean puncture times was 2-3. Histologic examinations were finished in 140 cases. The satisfactory rate of sampling 16G and 18G were 100% and 97.1%, respectively. There were no serious complications in all cases. The sensitivity of US-CNB in the diagnosis of SOBL was 98. 0%, and the Youden's index was 0.98. However, the sensitivity and the Youden's index of US in that were 83.7% and 0.79, respectively. Conclusion US-CNB is sensitive and accurate in the diagnosis of SOBL with a lower incidence of complication, it is an effective technique in confirming the diagnosis of SOBL.
