组织多普勒测量三尖瓣环运动评估肺动脉压的初步探讨 被引量:1

Noninvasive Evaluation of Pulmonary Artery Pressure by Tricuspid Annulus Velocity Parameters Derived from Doppler Tissue Imaging:Primary Study

摘要 目的探讨用组织多普勒技术测量三尖瓣环运动来评估心脏病患者肺动脉压的可行性。方法以组织多普勒技术分别测量46例心脏病患者和41例健康体检者三尖瓣环的运动参数,同时以漂浮导管测量心脏病患者的肺动脉压,推导肺动脉压与三尖瓣环间的回归方程,并对回归方程进行综合评价。结果得出回归方程PAMP=39.377×(Va/Ve)+0.132(Ds-De)-12.732VTIe+14.286,其诊断肺动脉高压的灵敏度为93%、特异度为72%、阳性预测值为78%、阴性预测值为86%、正确率为83%。结论上述回归方程能可靠地评价肺动脉平均压。 Objective To evaluate pulmonary artery pressure in patients with heart diseases with pulsed wave Doppler tissue imaging. Methods Doppler tissue imaging at tricuspid annulus was performed in 46 patients with heart diseases and 41 healthy volunteers. The patients underwent cardiac catheterizalion simultaneously and pulmonary artery pressures were obtained. Fifty four parameters of Doppler tissue imaging at tricuspid annulus were compared with pulmonary artery pressure detected by catheterization and muhiple regression equations were derived. Results The multiple regresstnn equations were deduced: PAMP= 39. 377 × (Va/Ve) + 0. 132 (Ds-De) 12. 732VTIe + 14. 286. Using this formula, the prediction of PAMP〉25 mm Hg was calculated and the results were as follows: specificity of 72%, sentivity of 93%, positive predictive value of 78%, negative predictive value of 86% and accuracy of 83%.Conclusions The above multiple regression equation derived from Doppler tissue imaging at tricuspid annulus could be used to evaluate the pulmonary artery pressure creditably.
