不同糖耐量人群中血清脂联素水平变化及与大血管病变的关系 被引量:2

Relationship between serum adiponectin levels and macrovascular pathological changes in different glucose tolerance humans

摘要 目的检测不同糖耐量人群中血清脂联素水平,探讨血清脂联素水平与2型糖尿病大血管病变的关系。方法选择新确诊的2型DM患者(DM组)84例,糖耐量减低患者(IGT组)46例,以体检人群中无糖代谢紊乱个体50例(NGT组)作为对照组。各组按体重指数(BMI)分成肥胖和非肥胖亚组,分别测定血清脂联素水平。以双侧颈总动脉、颈外动脉内膜-中层厚度(IMT)≥1.0mm或局部有斑块者诊断为颈动脉粥样硬化,分析血清脂联素水平与2型糖尿病大血管病变的关系。结果IGT组脂联素水平较NGT组明显降低(P<0.01),DM组降低更显著(P<0.01);肥胖亚组中血清脂联素水平低于非肥胖亚组(P<0.01);合并大血管病变者血清脂联素水平低于无大血管病变者(P<0.01)。结论血清脂联素降低在2型糖尿病发病中以及和大血管病变的发生有密切关系。 Objective To investigate the relationship between serum adiponectin (ADI) levels and macrovascular pathological changes in patients with type 2 diabetes mellitus. Methods 84 patients with type 2 diabetes mellitus (DM group) and 46 cases of impaired glucose tolerance (IGT group) were chosen in the study, and 50 healthy subjects were chosen as normal glucose tolerance group (control group). Each group was divided into two subgroups based on BMI, the serum adiponectin levels were detected. ITM ± 1.0 mm or local spot was diagnosed as cervical artery AS and the relationship between serum adiponectin levels and type 2 diabetes mellitus was analyzed. Results The serum adiponectin levels in IGT group were significantly lower than those of control group ( P 〈 0.01 ). The serum adiponectin levels in patients with type 2 diabetes mellitus were significantly lower than those in IGT group ( P 〈 0. 01 ). The ADI levels in obesity group were obviously lower than those of non-obesity group ( P 〈 0.01 ), the levels of ADI in patients with macrovascular pathological changes were significantly lower than those without macrovascular pathological changes ( P 〈 0.01) .Conclusion The decrease of serum adiponecin concentration has a close relationship with the occurrence of type 2 diabetes mellitus and macrovascular pathological changes.
