余氏调补活血法治疗慢性萎缩性胃炎综合疗效评价

Comprehensive Evaluation of Professor Yu's Spleen-Strengthening,Stomach-Regulating and Blood-Activating Therapy for Chronic Atrophic Gastritis

【目的】综合评价余氏调补活血法治疗慢性萎缩性胃炎的临床疗效。【方法】总结近3年运用具有调补活血功效的萎胃复元汤(北芪、太子参、生地、三七、五灵脂、半枝莲、白花蛇舌草、枳实、延胡索等)加减治疗32例轻、中、重度慢性萎缩性胃炎患者,分别从临床症状、胃镜、病理组织、幽门螺杆菌(HP)清除等方面进行治疗前后对比,并进行综合疗效评价。【结果】经3个月至半年治疗后患者的临床症状均有明显改善;胃镜检查结果显示,患者的胃粘膜萎缩较治疗前有显著性改善(P<0.01),胃粘膜组织病理学检查,治疗后患者的胃粘膜腺体萎缩、肠上皮化生和不典型增生均较治疗前有显著性改善(P<0.05或P<0.01);治疗后患者的HP感染得到有效控制,与治疗前比较差异有显著性意义(P<0.05);综合疗效评价结果为:显效18例,有效7例,无效7例,显效率为56.25%,总有效率为78.13%。【结论】余氏调补活血法治疗慢性萎缩性胃炎有良好临床疗效。

[ Objective ] To comprehensively evaluate the therapeutic effect of Professor Yu＇s spleen-strengthening, stomach-regulating and blood-activating therapy for chronic atrophic gastritis （CAG）. [ Methods] Thirty-two CAG patients collected in recent 3 years were treated with Weiwei Fuyuan Decoction （WFD）, a prescription embodying Professor Yu＇s spleen-strengthening, stomach-regulating and blood-activating therapy. WFD is mainly composed of Radix Astragali, Radix Pseudostellariae, Radix Rehmanniae, Radix Notoginseng, Faeces Trogopterori, Herba Scutellariae Barbatae, Herba Hodyotis Diffusae, Fructus Aurantii Immaturus, Rhizoma Corydalis, etc.. CAG was classified into mild, moderate and severe degree in this study. The treatment lasted 3 months to half a year. After treatment, the effect of WFD on clinical symptoms, gastroscope, pathological changes, helicobacter pyloria （liP） infection was observed and compared to that before treatment. [ Results] After treatment, the clinical symptoms were much relieved, and atrophy of gastric mucosa （GM） under gastroscope was also relieved （P 〈 0.01） as compared to that before treatment. Meanwhile, pathological damage such as gland atrophy of GM, intestinal metaplasia of GM and atypical hyperplasia of GM were reduced （ P 〈 0.01 or P 〈 0.05）, and HP infection was effectively controlled （ P 〈 0.05）. The results of comprehensive evaluation showed that 18 patients were markedly effective, 7 effective, 7 ineffective and the markedly effective rate was 56.25 %, the total effective rate being 78.13 %. [ Conclusion] Professor Yu＇s spleen-strengthening, stomach-regulating and blood-activating therapy is effecitve for CAG.
