彩色多普勒超声检测异位妊娠患者卵巢黄体血流动力学变化的临床价值

The application value of luteinic blood by color Doppler flow imaging for diagnosing ectopic pregnancy

目的探讨彩色多普勒(CDFI)超声检测异位妊娠卵巢黄体血流变化的临床价值。方法1998-02-2005-10广州医学院第二附属医院和广东增城市人民医院运用CDFI分别对20例胚胎存活型异位妊娠,36例流产型异位妊娠和45例宫内妊娠孕妇黄体血流动力学参数(收缩期峰值流速SPV、阻力指数RI)进行统计学处理和对照分析,并观察卵巢内黄体血流分布特点。结果胚胎存活型异位妊娠组黄体血流峰值、血流阻力指数与宫内妊娠组比较无明显差异。流产型异位妊娠组黄体血流峰值明显低于宫内妊娠组及胚胎存活型异位妊娠组(P<0.01),而血流阻力指数则高于宫内妊娠组及胚胎存活型异位妊娠组(P<0.01)。结论CDFI检测卵巢黄体血流对诊断是否存在异位妊娠无明显意义,但对异位妊娠中胚胎是否存活具有较高的诊断价值。

Objectives To evaluate the clinical value of luteinic blood flow with color Doppler flow image（CDFI） on ectopic pregnancy. Methods The blood dynamic parameter of 20 cases of live embryo ectopic pregnancy ,36 cases of abortive ectopic pregnancy and 45 cases of uterine pregnancy from Feb. 1998 to Oct. 2005 in the Second Affiliated Hospitl of Guanzhou Medical University and the Peopleg Hospital of Zengcheng were determined with color Doppler flow image and statistically analyzed, and the characteristics of luteinic blood distribution were observed. Results Systolic peak velocity and resistense index of luteinic blood flow were not different between the live embryo ectopic pregnancy and uterine pregnancy group. Systolic peak velocity of luteinic blood flow in abortive ectopic pregnancy group was lower than that of uterine pregnancy and of live embryo ectopic pregnancy group （P 〈 O. O1 ）. Resistense index was higher than that of uterine pregnancy and of live embryo ectopic pregnancy group （ P 〈 O. O1 ）. Conclusion The luteinic blood flow with color Doppler flow imaging is valuable for diagnosing whether or not there is live embryo in ectopic pregnancy, but it is not valuable for diagnosing whether or not there is ectopic pregnancy.
