尿毒症血液透析病人血清脂联素与血脂水平的关系 被引量:1

摘要 目的:探讨尿毒症血液透析病人血清脂联素(ad iponectin)水平的变化;了解血液透析病人血脂水平的变化以及脂联素与血脂的相关性,分析脂联素对血脂的影响。方法:应用EL ISA法测定30例体检健康者与76例慢性肾衰尿毒症血液透析病人血清脂联素的水平;全自动生化分析仪测定血脂,包括甘油三酯(TG)、总胆固醇(TCh)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、脂蛋白a[Lp(a)]。结果:尿毒症组病人血脂联素、TG、Lp(a)水平明显高于健康对照组(P<0.05或P<0.01)。尿毒症组病人血HDL-C明显低于健康对照组(P<0.05)。尿毒症病人血清脂联素与TG呈负相关,相关系数-0.42(P<0.01);与HDL-C呈正相关,r为0.33(P<0.05)。脂联素与TCh、LDL-C、Lp(a)无显著性相关。结论:尿毒症血液透析病人体内存在脂质代谢紊乱,表现为TG、Lp(a)水平升高,HDL-C水平下降。脂联素可能通过干预脂质代谢对尿毒症血液透析病人具有保护作用。 Objective: To explore the serum adiponectin and blood lipid levels in ureamic patients undergoing hemodialysis compared with those of healthy controls, the relationship between the both and the effect of adiponectin on blood lipid and atherosclerosis. Methods: Serum adiponectin level was measured in 76 chronic hemodialysis patients with uremia and 30 healthy controls by ELISA and the blood lipid levels were determined with automatic biochemistry analyzer including triglyceride, total cholesterol, high density lipoprotein cholesterol, low density lipoprotein cholesterol and lipoprotein(a). Results: The levels of serum adiponectin, triglyceride and lipoprotein(a) in uremic patients were significantly higher (P 〈 0.05 or P 〈0. 01) , while high density lipoprotein cholesterol was lower , compared with the control (P 〈 0.05) , showing negative correlation with triglyceride, r = -0. 34, P 〈 0.01 ), and positive correlation with high density lipoprotein cholesterol ( r = 0.33, P 〈 0.05) . Conclusion : There are blood lipid abnormalities in ureamic patients, and adiponectin may be a protective factor by intervention of blood lipid metabolism.
