机械通气救治4例银环蛇咬伤急性呼吸衰竭

Mechanical Ventilation for Four Patients with Acute Respiratory Failure by Bungarus multicinctus Blyth Snakebite

目的 探讨机械通气救治银环蛇咬伤致急性呼吸衰竭的治疗效果。方法 院前正确清创急救处理，及时行机械通气救治4例银环蛇咬伤致急性呼吸衰竭患者。结果 气管内插管是最佳的气道开放方法，气管插管者辅助通气4～6h后撤机及拔管可获得成功。结论 正确急救和清创，气管内插管及机械通气是救治银环蛇咬伤致急性呼吸衰竭的最有效的方法。

Objective To assess the therapeutic results of mechanical ventilation for acute respiratory failure （ARF）caused by Bungarus rnulticinctus Blyth Snake bite. Methods Respirator was used to deal with 4 patients with this disease. Results Our cases analysis showed that the intubation in this critical conditions had the best of effects and benefits for opening the patient airway;with endotrachea intubation, assisted ventilation in 4～6h can be successful in weaning and extubating in these patients. Conclusion Mechanical ventilation through endotrachea intubation is the most effective method to recover the ARF caused by Bungarus rnulticinctus blyth snakebite, and the perfect debridements in first aid are very important.
