心脏病患者血清天门冬氨酸氨基转移酶及其同工酶检测的临床意义

目的探讨心脏病患者血清中天门冬氨酸氨基转移酶(AST)和线粒体同工酶(m-AST)的变化及其临床意义。方法选择住院心脏病患者54例,其中先心病15例,风心病20例,冠心病19例,正常对照组30例。采用速率法测定血清中AST、m-AST含量并计算m-AST/AST比值。结果冠心病组、风心病组、先心病组m-AST含量均高于对照组(P<0.05,P<0.01)。风心病组、先心病组AST含量、m-AST/AST值显著高于对照组(P<0.01)。冠心病组m-AST高于对照组(P<0.05),冠心病组与风心病组相比较AST差异有统计学意义(P<0.05)。结论m-AST、m-AST/AST值测定与心肌细胞损伤程度有关,可作为心脏疾病的辅助诊断指标。

Objective To investigate the changing way of serum m-AST and m-AST/AST ratio in patients with heart disease （HD） as well as their clinical significance. Methods To choose fifty four patients with congenital heart disease （CHD, 15 cases）, rheumatic heart disease （RHD,20 cases）, coronary atherosclerosis disease（CAD, 19 cases） and normal control （30 cases）. By using velocity assay, the activities of the patients＇serum m-AST and AST were measured. The m-AST/AST ratio was calculated. Results The value of m-AST was all elevated in patients with CHD, RHD and CAD. （P〈0.05 or P〈0.01）. The value of m- AST and the m-AST/AST ratio in patients with CHD and RHD had significantly difference compared with normal control. Besides, the difference of the value of AST in patients between CAD and RHD was notable （P〈0.05）. Conclusion The detections of m-AST＇s activity and the m-AST/AST ratio can evaluate the level of myocardial injury and be considered as a useful index of laboratory diagnosis for HD.
