不同术式及术前血糖浓度对糖尿病患者白内障手术的影响

Effect of Different Blood Sugar Concentrations and Operation Method on Cataract Surgery Patients with Diabetes

【目的】探讨术前血糖浓度及术式对糖尿病患者白内障人工晶体植入手术的影响。【方法】将伴有糖尿病的白内障患者56例65眼,根据术前血糖情况分为理想控制、较好和一般控制,控制不理想三组。接受白内障囊外摘除术43眼,白内障超声乳化抽吸术22眼,两种术式均联合囊袋内植入人工晶体。【结果】糖尿病组中血糖控制理想和血糖控制一般(范围控制在8.33 mmol/L内)的病人白内障手术后明显房水闪光的例数与对照组比较差异无显著性;血糖控制不理想的病人术后明显房水闪光的例数与对照组比较差异有显著性。分别接受ECCE手术和Phaco手术的糖尿病患者术后出现明显房水闪光的例数比较差异无显著性。【结论】糖尿病患者在实施白内障手术前稍高的血糖并不影响手术。但术前血糖水平过高会增加术后眼内炎症反应,应给予应有的重视。伴糖尿病的白内障患者在实施ECCE术式与超声乳化术式在手术后房水闪光比较差异无显著性,说明术式对白内障手术的术后反应影响不大。

[Objective]To study the effect of different blood sugar concentrations and operative method on cataract surgery of patients with diabetes. [Methods]Fifty-six cataract patients （65 eyes） with diabetes were divided into three groups according to different blood sugar concentration. All of them had an operation with extracapsular cataract extraction（ECCE） or Phacoe mulsification. [Results]The patients with normal and slightly higher blood sugar had no difference in aqueous flare comparing with control group. The patients with excessively higher blood sugar（〉8.33mmool/L） had significantly different flash comparing with control group. The aqueous flare in patients operated with ECCE and Phacoe had no difference. [Conclusion]Normal and slightly higher blood sugar before operation does not affect blood sugar could make severe aqueous flare. The operative in anterior aquae of patients after ECCE or Phacoe. cataract operative result, but excessively higher method also does not affect inflammative reaction.
