微型侧壁螺丝刀在下颌骨内固定中的应用

An application of micro-angular screwdriver to internal fixation of mandibular fracture

目的评价微型侧壁螺丝刀在下颌骨角部和髁状突颈部骨折内固定中的临床意义。方法对21例下颌骨角部和髁状突颈部骨折患者,采用微型侧壁螺丝刀,经口内微型钛板内固定。结果术后随访3～15个月(平均10个月),所有患者均对面部外形满意。19例恢复正常咬合关系,2例轻微错(牙合),张口度均达到3cm以上。复查曲面断层片或螺旋CT三维重建,下颌骨骨折对位良好。结论采用微型侧壁螺丝刀经口腔内固定下颌骨角部和髁状突颈部骨折,完全避免了颌面部皮肤切口,尤其对患者的心理安慰,意义重大。

Objective To investigate the internal fixation of mandibular angle and condyle fracture with a microangular screwing instrument. Methods In 21 cases, internal fixations were performed at mandibular angle and condyle fractures with a micro-angular screwdriver through intraoral minimally invasions. Results All of the patients were satisfied with their curative effects.The mean follow-up period was 10 months （3 to15 months）,19cases were reinstated to normal occlusions,and slight malocclusions existed in 2 cases.Postoperative OPTs and spiral CT scan three dimensional reconstructions demonstrated that all fractures were well reduced. Conclusions Intraoral internal fixation on mandibular angle and condyle fracture with a micro-angular screwdriver avoided facial incisions thoroughly It was of great significance to the patients.
