颈淋巴结清扫在甲状腺癌再手术中的价值

Neck dissection for thyroid carcinoma patients who received nonstandard operation

目的探讨颈淋巴结清扫术(简称颈清扫术)在甲状腺癌不规范手术后再手术中的价值及术式选择。方法回顾性分析1997年至2005年间,38例甲状腺癌不规范手术后再次手术患者中,32例作颈清扫术者的临床资料。其中,功能性颈清扫+患侧残叶+峡部切除24例,功能性颈清扫术+全叶切除术2例,单纯行功能性颈清扫术4例,区域性颈清扫术2例。结果颈清扫术淋巴结阳性率68.75%(2/232),甲状腺肿瘤残留率47.38%(18/38),总癌残留率73.68%(2/838)。结论甲状腺癌不规范手术的颈淋巴结肿瘤残留率较高,颈清扫术在甲状腺癌再次手术中具有明确的治疗作用。

Objective To study the values of neck dissection for thyroid carcinoma who received nonstandard operation and help to choose good re-operation methods. Methods Retrospective investigation was carried out in 38 cases of neck dissection for thyroid carcinoma patients who received nonstandard operation during 1997 to 2005, and 32 cases were treated with neck dissection. Results Pathological results con- firmed there were 73.68 % with residual tumor, positive rate of thyroid was 47.38 %, and that cervical lymph node was 57.89 %. Conclusion The residual rate of cervical lymph nodes of reoperated patients with thyroid carcinoma was relatively higher, so neck dissection for thyroid carcinoma who received nonstandard operation was necessary.
