脑疝致自主呼吸停止不同时间窗脑组织病理变化的实验研究

摘要 目的通过硬膜下球囊注水导致脑疝形成致犬自主呼吸停止,观察脑疝致自主呼吸停止不同时间窗不同部位脑组织的病理改变。方法Beagle犬40只,随机分为正常组、假手术组(只开骨窗不行硬膜下球囊注水)、手术组。通过右侧颞部颅骨钻孔硬膜下球囊注水致Beagle犬自主呼吸停止后,立即给予气管插管及辅助呼吸,并在自主呼吸停止后不同时间窗(15min组、30min组、45min组、60min组)行去除水囊及骨窗扩大术。观察犬在不同时间窗不同部位脑组织的病理改变。结果光镜显示:15min组不同部位脑组织均表现为轻度充血,30min组不同部位脑组织充血水肿加重,45min组脑组织出现神经细胞变性坏死,60min组脑组织结构破坏严重。超微结构的研究显示:45min组脑细胞结构尚清楚,线粒体等细胞器水肿,髓鞘板层结构厚薄不一;60min脑细胞结构不完整,包膜、核膜、线粒体等崩解,神经元胞体碎裂,血管周细胞坏死明显。结论脑疝致自主呼吸停止时间越长脑组织病理改变越明显、脑组织变性坏死越严重。 Objective To observe the pathological changes in different brain tissues in different time after respiratory arrest induced by brain hernia through injecting water to subdural balloon. Methods 40 Beagle dogs were divided into normal grounp ,contral grounp ,and operation grounps in random. In operation grounps respiratory arrest were made by injecting water to right subdural balloon,then endotracheal intubation with breathing was assisted by a respirator. And take off balloon and extend bony cavity in different times (15 minutes, 30 minutes ,45 minutes and 60 minutes) after respiratory arrest. Then we observed the pathological changes of different brain tissues in different time. Results Under light microscope, we saw the different changes:light hyperaemia in the 15min groups,and more in the 30min,the nerve cells in brian show necrosis degeneration in 45min groups,while the cellular figure was damaged seriously in 60min groups. Electron microscope showed the cellular figure was exist with mitochondrion and others cellular Organelles swell ,myelin sheath thickness and thinness mixed in 45 minute groups. The cellular figure was damaged completely including all organelles in the 60 minute groups. Conclusion The longer time of respiratory arrest caused by brain hernia ,the more severe damage of brain tissue.
