腹腔镜下广泛全子宫切除加盆腔淋巴结清扫治疗早期子宫颈癌25例临床研究

Clnical analysis of 25 cases of laparoscopic radical hysterectomy lymphadenectomy for early-stage cervical cancer

目的:分析腹腔镜下广泛全子宫切除及盆腔淋巴结清扫治疗早期子宫颈癌的可行性及疗效。方法:回顾分析腹腔镜手术治疗早期子宫颈癌25例临床资料,总结其手术时间、术中出血量、淋巴结切除数目、手术并发症及近期预后情况。结果:中转开腹1例,余24例均在腹腔镜下完成手术。手术时间为(156·74±47·11)min,术中出血量(163·48±107·26)ml,淋巴结切除数(16·47±6·58)个,术中脏器损伤发生率8%,手术并发症发生率24%。术后随访22例,其中死亡1例(与复发有关),余均未复发。结论:腹腔镜下广泛全子宫切除及盆腔淋巴结清扫治疗早期子宫颈癌安全、可行,近期疗效好,远期疗效有待进一步研究。

Objective ： To analyze the feasibility and curative effect of laparoscopic radical hysterectomy and pelvic lymphadenectomy for early - stage cervical cancer. Methods： A retrospective analysis was carried out on the clinical data of 25 cases cervical cancer which accepted laparoscopic surgery, and a summary was made on the operating time, blood loss in operations, the amount of the excised lymph nodes, the operation complications, and the recent prognosis. Results： All operations were successfully performed under laparoscopy except 1 case was opened the abdomen during the operation. The operating time was （ 156. 74 ± 47.11 ） min, blood loss in operation was （ 163.48 ± 107.26） ml, the amount of the excised lymph nodes was （16. 47 ± 6. 58 ）, the visceral traumatic rate was 8%, and the postoperative complication rate was 24%. Of postoperative followed up 22 cases, 1 case died, the rest did not recur. Conclusion： Laparoscopic redical hysterectomy and pelvic lymphadenectomy for early stage cervical cancer is safe and feasible. It has a satisfactory recent curative effect, but an advanced research should be made on the long -term curative effect.
