摘要
目的探讨梗死前心绞痛对急性心肌梗死（AMI）病人心率变异性（HRV）的影响。方法选择1999年1月-2003年12月我院心内科收治的AMI病人208例，按有无梗死前心绞痛史分为A（无心绞痛史）、B（有心绞痛史）2组。比较两组病人HRV、恶性室性心律失常发生率、病死率及死亡原因。结果HRV各项指标如NN间期的标准差、短时程平均NN间期的标准差、连接NN间期均方差值的均方根、全程记录中每5rainNN间期标准差的平均值，B组均高于A组（t=2．862～3．675，P〈O．01）。病死率及恶性室性心律失常发生率，A组高于B组（X^2=7．79、7．18，P〈O．01）。死亡原因无明显差别。结论梗死前心绞痛能使AMI病人HRV增大，同时能够降低恶性室性心律失常的发生率。
Objective To investigate the influence of pre-infarction angina pectoris on heart rate variability in patients with acute myocardial infarction. Methods A total of 208 consecutive patients with acute myocardial infarction were divided into group A （without previous angina） and group B （with angina）. A comparison was made between the two groups in terms of heart rate variability （HRV）, vicious ventricular arrhythmia, fatality rate, and causes of death. Results Each index of HRV was lower in group A （t=2. 862-3. 675, P〈0.0l）. The incidence of vicious arrhythmia was higher in group A （X^2 = 7. 18, P〈 0. 01）. The in-hospital death was higher in group A （X^2 = 7. 79, P〈0.0l）, while the causes of death were not significantly different between the two groups. Conclusion Pre-infarction angina pectoris can increase the heart rate variability in patients with acute myocardial infarction and reduce the incidence of vicious ventricular arrhythmia.
