血清CEA、CA199及CA242在消化系统肿瘤组织中的表达

EXPRESSIONS OF SERUM CEA,CA199 AND CA242 IN CARCINOMAS OF DIGESTIVE SYSTEM

目的探讨血清癌胚抗原（CEA）、糖链抗原19-9（CA19-9）及糖类抗原CA242在胃癌、原发性肝癌、胰腺癌及结直肠癌等消化系统肿瘤中的表达水平及临床意义。方法应用ELISA法检测26例食管癌、49例胃癌、68例原发性肝癌、32例胰腺癌、30例胆管癌、67例结直肠癌、55例消化系统良性病变和47例正常人血清CEA、CA19—9、CA242的表达水平。结果各肿瘤组血清CEA、CA19-9、CA242阳性表达率显著高于良性病变组和正常对照组（X^2=86．54～113．29，P〈0．05）。CA242在食管癌和胃癌中表达水平最高，CA19-9在原发性肝癌、胰腺癌和胆管癌中表达水平最高，CEA在结直肠癌中表达水平最高。结论消化系统肿瘤因部位不同，其血清CEA、CA19-9、CA242的阳性表达率也不一致，临床应有针对性地选择肿瘤标志物。

Objective To assess the expressions and the clinical significance of serum CEA, CA199 and CA242 in carcinomas of digestive system. Methods The expression of serum CEA, CA199 and CA242 were detected with ELISA assay in carcinoma of esophagus （26 cases）, stomach （49）, liver （68）, pancreas （32）, bile duct （30）, colon or rectum （67）, benign diseases in digestive system （55） and normal controls （47）. Results The expression rates of CEA, CA199 and CA242 in cases with various carcinomas were significantly higher than those without （X^2 = 86.54- 113.29, P〈0.05）. The patients with esophageal and gastric carcinomas had the highest CA242 expression rate. Those with primary liver, pancreatic and bile duct carcinomas had the highest CA199 expression rate; and those with colon or rectal cancers had the highest CEA expression rate. Conclusion The expressions of serum CEA, CA199 and CA242 in carcinomas of digestive system differ with the different tumor locations. Clinically, the selection of tumor marker should be directed to different conditions.
