耐甲氧西林葡萄球菌的多重PCR快速检测 被引量:3

摘要 目的采用多重聚合酶链反应(PCR)检测耐甲氧西林葡萄球菌(MRS),并对我院葡萄球菌的耐药率进行调查。方法对262株葡萄球菌(金黄色葡萄球菌86株,凝固酶阴性葡萄球菌176株)进行多重PCR扩增,并与琼脂稀释法作比较。对我院葡萄球菌的耐药率进行统计分析。结果以mecA基因阳性判断MRS,灵敏度为100%,特异度为96.6%,阴性预测值为100%,阳性预测值为99.0%;以mecA基因和femA基因阳性判断耐甲氧西林金黄色葡萄球菌(MRSA),灵敏度为100%,特异度为99.5%,阴性预测值为100%,阳性预测值为98.5%。我院葡萄球菌耐药率为78.6%,其中凝固酶阴性葡萄球菌耐药率为79.0%,金黄色葡萄球菌耐药率为78.0%。结论多重PCR技术检测mecA基因能快速、敏感、准确地检测MRS,与femA基因联合检测能有效检测MRSA。MRS已成为日益严重的葡萄球菌感染问题。 Objective To detect methicillin-resistant staphylococcus (MRS) by multiple PCR, and to investigate its drug resistance rate in our hospital. Methods The DNA fragments of 262 strains of staphylococcus (staphylococcus aureus, 86; coagulase-negative staphylococcus, 176) were amplified by multiple PCR. The results were compared with Agar Dilution. The drug resistance rate was analyzed statistically. Results Taking mecA positive for MRS detection, the sensitivity,specificity,negative predictive values and posftive predictive value were 100%,96.6%,100% and 99.0%, respectivdy;taking mecA positive and femA positive for methicillin-resistant staphylococcus aureus (MRSA) detection, the sensitivity, specificity, negative predictive values and positive predictive value were 100 %;, 99.5 %, 100% and 98.5 %, respectively. The drug resistance of staphylococcus in our hospital was 78.6%, in which, coagulase-negative staphylococcus accounting for 79.0%, and staphylococcus aureus, 78.0%. Conclu- sion The detection of mecA gene by multiple PCR for MRS is sensitive,rapid and accurate. A combination of mecA and FemA can effectively detect MRSA. MRS has become an increasingly serious problem in staphylococcus infection.
