参附注射液对肝缺血再灌注大鼠微循环的影响 被引量:9

摘要 目的:应用大鼠肝缺血再灌注(IR)损伤模型探讨参附注射液对肝微循环的影响。方法:12只W istar大鼠随机分为IR组和SF组。SF组给予参附注射液10 m l/kg。IR组大鼠给予相同剂量的生理盐水。两组均采用Pringle法阻断肝门缺血15m in再灌注3 h,测定血清生化酶ALT、AST、LDH和血浆TXB2、6-keto-PGF1α,及观察肝组织形态学改变。结果:SF组血清ALT、AST、LDH水平低于IR组;血浆TXB2低于IR组,6-keto-PGF1α高于IR组,二者比值TXB2/6-keto-PGF1α低于IR组;SF组肝实质细胞和肝窦内皮细胞损伤明显减轻,肝窦形态的改变和肝窦内的瘀血、白细胞聚集也明显减轻。结论:参附注射液通过保护肝窦内皮细胞,降低TXA2/PG I2比值及减轻肝窦内炎性反应,改善肝脏微循环灌注。 Objective: To explore the effect of Shenfu injection on the microcirculation during hepatic ischemia and reperfusion in rats. Methods: Twelve male Wistar rats weighing 200 to 250 g were randomly divided into 2 groups : SF group ( rats were intraperitoneally injected with Shenfu injection of 10 ml/kg) and IR group ( rats were injected with 0.9% sodium chloride solution at the same dose of Shenfu injection). Hepatic ischemia was caused by Pringle' s maneuver and lasted for 15 minutes, and then 3-hour reperfusion was performed. Arterial blood samples for liver function tests and venous blood samples for the measurement of thromboxane B2 ( TXB2 ) and 6-keto-PGF1α were collected 3 hours after reperfusion. Liver tissues were sampled 3 hours after reperfusion and fixed in 2.5% glutaraldehyde solution and 10% formalin solution respectively for morphological study. Results: Compared with IR group, in SF group, the levels of liver enzymes ( alanine aminotransferase, aspirate aminotransferase, and lactate dehydrogenase) in serum, the level of TXB2 in plasma, and the ratio of TXB2 to 6-keto-PGF1α were lower, while the level of 6-keto-PGF1α in plasma was higher. Histopathological examination showed that 3-hour reperfusion after 15-minute ischemia caused severe hepatic histological alterations. Marked structural abnormalities were observed in IR group. In SF group, hepatic tissue injury was significantly reduced. Conclusion: Shenfu injection protects hepatic tissues from ischemia-reperfusion injury by defending sinusoidal endothelial cell, alleviating leukocyte aggregation, and reducing the ratio of thromboxane A2 to prostacyclin, and thus improves hepatic microvascular perfusion.
