Transurethral resection of the prostate by modified Blandy′s method in large-sized and high-risk benign prostatic hyperplasia

目的:探讨改良B landy法经尿道前列腺切除术(TURP)治疗高危重度前列腺增生症(BPH)的临床效果。方法:高危重度BPH 49例,在有效治疗原有内科疾病基础上,B landy法TURP治疗17例,改良B landy法TURP治疗32例,对两组病例的手术时间、切除腺体重量、术中出血量、术中输血量等进行比较。结果:全部病例治疗过程顺利,效果良好。B landy法TURP切除腺体平均56 g,平均手术时间85 m in,术中平均出血282 m l,术中输血10例,平均输血400 m。l改良B landy法TURP切除腺体平均57 g,平均手术时间70 m in,术中平均出血210 m l,术中输血15例,平均输血300 m。l结论:改良B landy法TURP治疗高危重度BPH手术时间短、术中出血少,安全、可行。

Objective： To explore the clinical efficacy of the transurethral resection of the prostate （TURP） by modified Blandy＇s method in large-sized and high-risk benign prostatic hyperplasia （BPH）. Methods： Among 49 patients with severe BPH, 17 received TURP by Blandy＇s method and 32 received TURP by modified Blandy＇s method. The operative time, bleeding volume during operation, and blood transfusion volume were compared between the patients. Results： The operations were successful. In patients receiving TURP by Blandy＇s method, the mean weight of the resected prostate tissue was 56 g; the mean operative time was 85 minutes; the mean bleeding volume during operation was 282 ml; the mean bloed transfusion volume was 400 ml. In patients received TURP by modified Blandy＇s method, the mean weight of the resected prostate tissue was 57 g; the mean operative time was 70 minutes; the mean bleeding volume during operation was 210 ml; the mean blood transfusion volume was 300 ml. Conclusion： TURP by modified Blandy＇s methods is a safe and effective method. It could improve the quality of operation for large-sized and high-risk BPH, with less operative time and less bleeding volume during operation.
