颈前路术后钛网下沉对颈椎曲度和手术疗效的影响 被引量:14

Influence of Titanium Mesh Subsidence on Cervical Curvature and Surgical Effect after Anterior Cervical Corpectomy Reconstructed with Titanium Mesh and Bone Graft

摘要 目的分析颈前路钛网植骨融合术后钛网下沉对颈椎曲度和手术疗效的影响。方法回顾性分析24例颈前路钛网植骨融合术后钛网发生下沉,测量手术前后及钛网下沉后颈椎曲度“D”值,JOA评分法评价手术前后及钛网下沉后神经功能改变情况,并分别行统计学比较。结果所有病例术后JOA评分平均增加6·5分(P<0·01),发生钛网下沉后18例JOA评分平均增加1·8分,6例平均下降2·1分,但结果无统计学意义。术后即刻颈椎曲度“D”值平均提高(9·25±2·52)mm(P<0·05),钛网下沉后视发生部位的不同“D”值增减不一,结果均无统计学意义。结论颈前路术后钛网下沉不影响颈椎曲度和手术疗效。 Objective To analyze the influence of the titanium mesh subsidence on cervical curvature and surgical result after anterior cervical corpectomy reconstructed with titanium mesh and bone graft. Methods Twenty- four patients with subsidence after corpectomy were selected. Their neurological function was estimated pre - and post - operatively and after the subsidence of the titanium mesh by JOA grade and the change of cervical curvature was measured through the index of "D" value. Both were compared statistically. Results JOA grades in all patients were increased by 6.5 points averagely (P 〈 0.01 ) after operation. JOA grades were increased by 1.8 points in 18 patients and decreased by 2.1 points in 6 patients after the subsidence without significant difference. The postoperative immediate cervical curvature index of "D" value was increased by 9,25±2.52 mm averagely (P 〈 0.05) after operation. The "D" value was increased or decreased in different sites after the subsidence of the titanium mesh without the significant difference. Conclusion The titanium mesh subsidence has no effect on cervical curvature and surgical effect.
