血浆置换治疗重型肝炎常见并发症的临床分析

Clinical analysis of the common complications in plasma exchange for severe hepatitis

目的探讨使用血浆置换治疗重型肝炎患者出现的并发症及其治疗。方法利用日本KURARAY公司产的KM-8900型人工肝治疗仪治疗重型肝炎患者,治疗前详细了解患者的过敏史,治疗开始密切观察患者的生命体征,予适量氢化考的松预防过敏,统计分析血浆置换治疗重型肝炎86例193例次的并发症发生情况。结果治疗过程中共发生皮疹12例次(6.22%),血压一过性下降3例次(1.55%),血液管路部分凝血3例次(1.55%),下肢抽搐2例次(1.04%),无过敏性休克、畏寒、发热及出血发生,其并发症经及时处理后均治愈,不影响疗效。结论血浆置换治疗重型肝炎可发生多种并发症,但发生率很低,经对症处理后可治愈。

Objective To study the complications and its treatment in plasma exchange （PE） for severe hepatitis. Methods A total of 86 patients with severe hepatitis were treated by plasma exchange. Complications were occurred in 193 case times and analyzed. Results During PE treatment the complications were rash 12 case times （6.22 %）, transcient hypotention 3 case times （1.55 %）, lower extremety convulsion 2 case times （1.04 %）, patients show partial intravascular coagulation 3 case times （1.55 %）. Anaphylactic shock, fever, rigor and bleeding were not found. All of the complications were cured after treatment in time. Conclusion Although some complications may occur during PE treatment, but the occurrence is rather low and all of the complications will be curred after treatment in time.
