保留盆腔自主神经全直肠系膜切除术在直肠癌中的应用

The application of TME in autonomic nerve plexus preserving operation for rectal cancer

目的探讨保留盆腔自主神经全直肠系膜切除术(TME)及术后对患者泌尿生殖功能及根治效果的影响。方法采用病例对照的方法,回顾性分析了36例在直肠癌根治术中保留盆腔自主神经保留(PANP)组和非保留盆腔自主神经保留(非PANP)组病人31例术后性功能障碍的发生率以及局部复发率。结果研究组36例患者术后性功能和排尿功能指标优于对照组31例患者(P<0.05),两组术后局部癌肿复发率分别为8.3%和6.5%,差异无显著性(P>0.05)。结论保留盆腔自主神经全直肠系膜切除术在肿瘤根治和保持术后功能方面是适宜的术式。

Objective To evaluate the effects of TME with PNAP on the urinary , sexual fuctions and operation affections after operation for Rectal Cancer. Methods Retrospectively, the sexual dysfunction and local recurrence rate of 36 cases of TME with PANP and 31 cases without PANP were analysed by casecontral study Results The indication of sexual and urinary fuctions of those performed PANP（study group, n = 36）were better than those without PANP（contral group, n = 31 ）, （ P 〈 0.05）, the local recurrence rate of rectal cancer was 8.5 % and 6.5 % in study group and contral group respectively, but no significant difference was found between them （ P 〉0.05）. Conclusion TME with PANP is oncogically and fuctionally excellent and suitable for rectal cancer.
