米索前列醇与缩宫素防治产后出血的比较

Effects of misoprostol and oxytocin on postpartum hemorrhage

目的探讨米索前列醇预防和治疗产后出血的疗效。 方法对400例正常足月妊娠阴道分娩者，随机分为米索前列醇组（300例）和缩宫素组（100例），米索前列醇组在胎儿娩出后舌下含服米索前列醇200μg；缩宫素组在胎儿前肩娩出后静脉注射缩宫素10IU。 结果产后2h平均出血量米索前列醇组为138mL±26mL。缩宫素组为285mL±60mL（t9〈0．01）。产后出血发生率米索前列醇组为4．0％，缩宫素组为30％（P〈0．01）。米索前列醇组不良反应轻微。 结论米索前列醇是预防和治疗产后出血安全和有效的药物。

Aim To observe the effects in the prevention and hemorrhage. Methods Four hundred of misoprostol treatment of and oxytocin postpartum normal parturients were randomized into misoprostol group （n = 300 ） and the oxytocin group （n = 100）. The misoprostot methylate suppository was put under the tongue in misoprostol group The oxytocin group was given 10 IU oxytocin immediately after the delivery of anterior shoulder of the fetus. Results The amount of hemorrhage in 2 h postpartum was 138 mL ± 26 mL and 285 mL ± 60 mL, The difference between the 2 groups was statistically significant （P 〈 0. 01 ）. The postpartum hemorrhage rate in misoprostol group was 4.0%, and that in oxytocin group was 30%, which was statistically significant （P 〈 0.01） . The adverse reactions were mild in misoprostol group . Conclusion Misoprostol used in the prevention and treatment of postpartum hemorrhage is safe and effective.
