摘要
目的:探讨电针治疗腰椎间盘突出症的有效方法及其外周促循环镇痛机制。方法:将98例腰椎间盘突出症按入组先后随机分成治疗组(53例)与对照组(45例),分别给予局部单穴电针与常规取穴电针治疗(共8次),观察电针前后疼痛程度变化(共9次),以及血栓素B2血浆(TXB2)、前列环素(6-keto-PGF1α,6-K)和T/K比值的变化(共2次)。结果:两组治疗后患者疼痛程度均明显减轻(P<0.001),但治疗组比对照组更明显,存在着组间差异(P<0.001)。治疗结束后两组外周血TXB2和T/K比值均显著降低(P<0.001),而6-K则显著升高(P<0.001),各项指标治疗组比对照组变化更明显,存在着组间差异(P<0.001)。结论:局部单穴电针疗法对腰椎间盘突出症较常规取穴电针有更好的临床疗效,其作用机制可能与良性调节患者外周血TXB2、6-X及其T/K有关。
Objective:To study an effective therapy for the lumbocrural pain caused by lumbar inter- vertebral disc herniation ( LIDH ), and to explore the peripheral circulation-prompting analgesic mechanism of electroacupuncture in the treatment of LIDH, Methods:Ninety-eight patients of LIDH were randomly divided into treatment group (53 cases ) and control group (45 cases ). Single-point electroacupuncture stimulation (SPEAS) and conventional electroacupuncture stimulation (CEAS) were respectively used in two groups (total 8 times). The pain content ( SF-MPQ, 9 times ) and plasma TXB2,6-keto-PGF1a (6-K) , T/K ( 2 times ) levels were investigated respectively before and after treatment. Results:The results of SF-MPQ had significant difference than baseline from the second treatment (P 〈0. 001 ) in two groups. The changing range in the treatment group was more significant than that of the control group ( P 〈 0. 001 ) at the same time. The plasma TXB2, and T/K levels of all the patients are significantly decreased than those of baseline ( P 〈 0. 001 ) and the plasma 6-K level of two groups is significantly increased than those of baseline ( P 〈 0. 001 ) after 8 times treatment. The changing range in treatment group was more significant than that of control group ( P 〈 0, 001 ). Conclusions:SPEAS is more effective than CEAS in the treatment of LIDH and the mechanism may be related with the regulation effect of plasma TXB2 ,6-K and T/K level.
