转移皮瓣治疗会阴部Paget病(7例分析)

Reconstruction of defects in 7 vulvar Paget's diseases with flap transfer

目的探讨会阴部Paget病的临床特点与治疗。方法7例会阴部Paget病患者因手术切除病灶范围均较广,无法直接缝合,全部行皮瓣转移术。其中3例下腹壁带蒂皮瓣、2例腹股沟皮瓣、1例阴股沟皮瓣、1例阴囊中隔皮瓣。结果7例均一期手术成功,6例皮瓣成活,1例部分皮瓣坏死,后植皮成功。结论转移皮瓣是治疗会阴部Paget病手术切除病灶创面无法直接缝合的良好整形方案。

[Objective] To review our experience in the treatment of Paget＇s disease with flap transplantation. [Methods] The defects were all repaired by flaps in 7 patients with vulvar Paget＇s disease： 3 cases with low abdom- inal flap; 2 cases with inguinal flap; 1 case with pudendal - thigh flap; 1 case with scrotum flap. [Results] There were 6 cases of flap alive and 1 case in which 10 % or less of the skin paddle loughed and then skin graft was employed. [Conclusion] Flap transfer is the more ideal methods for reconstruction of the perineal region defects in Paget＇disease patients.
