急性左心衰合并慢性阻塞性肺病31例急救分析

Emergency treatment for acute left ventricular failure in 31 patients with chronic obstructive pulmonary disease

目的探讨急性左心衰合并慢性阻塞性肺病的诊断和治疗。方法对我院近4年收治的急性左心衰合并慢性阻塞性肺病31例急救情况进行回顾分析。结果31例急性左心衰合并慢性阻塞性肺病患者,临床表现以呼吸困难和肺部湿罗音为主。经吸氧、利尿、扩血管为主的综合治疗全部脱险。结论急性左心衰合并慢性阻塞性肺病多发生于老年人,以冠心病多见;利尿治疗效果良好。

[Objective] To explore the strategy of diagnosis and treatment for acute left ventricular failure in patients with chronic obstructive pulmonary disease （COPD）. [Methods] Emergency treatment analyzed retrospectively in 31 patients with acute left ventricular failure complicated with COPD. [Results] In all the patients, dyspnea and rales are prominent manifestation. All critical conditions improved significantly after administration of oxygen, diuretics and vasodilator. [Conclusions] Acute left ventricular failure, unusually caused by CAD and complicated with COPD is more common in older people, and diuretic treatment is effective.
