早发型重度子痫前期期待治疗期限探讨

Study of the duration of expectant management in early onset severe pre-eclampsia

目的：探讨早发型重度子痫前期期待治疗的期限对母儿结局的影响。方法：选择30例27—32孕周病情稳定而无合并症的患者进行期待治疗，期待治疗≤10d者16例（A组），〉10d者14例（B组），比较两组期待治疗的母儿结局。结果：30例入选患者，平均延长孕周9．14±6．13d；两组的确诊孕周无显著性差异（P〉0．05）；胎儿窘迫及小于胎龄儿发生率B组明显高于A组（P〈0．05）；围生儿死亡率A组明娃低于B组（18．75％ vs 33．33％，P〈0．05）。结论：对早发型重度子痫前期患者实施选择性期待治疗时，应严格评估母儿状况，治疗时间应有期限。

Objective： To explore the impact of the duration of expectant management on the fetaomaternal outcome in early onset severe pre- eclampsia. Methods： 30 cases with severe pre -eclamqsia at 27 N 32 weeks＇gestation were treated with expectant management. They were divided into two groups. In group A, the duration of 16 cases was 3 - 10 days. In group B, the duration of 14 cases was above 11 days. The fetaomaternal outcome of the two groups was compared. Results： The average pregnancy prolongation of all 30 study cases was 9. 14 ±6. 13 days. There was no different between two groups in the time of admission （ P 〉0. 05 ） . The rate of fetal distress and small for gestational age in group B were higher than those in group A （ P 〈 0. 05 ） . The incidence of perinatal death in group A was lower than group B （ P 〈 0. 05 ） . Conclusion： Sufficient and careful assessment is needed for the maternal and infant status before determining the expectant management for early severe pre - eclampsia. The duration of expectant management should be limited.
