应用免疫组化和免疫电镜观察61例大肠腺癌组织中胃泌素表达,同时应用放射免疫法检测血清胃泌素水平,结果36例(59%)呈阳性反应,高分化癌阳性率(13/16,81.3%)高于中分化癌(21/40,52.5%)和低分化癌(2/5,40.0%)。胃泌素表达强度与分化程度和Dukes分期有关。胃泌素阳性颗粒弥漫分布于癌细胞浆。免疫电镜胶体金颗粒(PAG)主要分布于癌细胞内不同电子密度分泌颗粒、癌细胞间隙和微绒毛膜表面。揭示癌细胞胃泌素表达是大肠癌分化良好的标志。癌细胞形成分泌颗粒释放胃泌素。血清胃泌素水平在癌组织胃泌素强阳性表达的病例中增高,可反映癌组织中胃泌素水平变化。
The expression of gastrin in human adenocarcinoma of large intestine was studied by immunohis-tochemistry and immunoelectron microscopy using protein A-gold complex (PAG). Of 61 patients tested, 36 cases(59.0% ) showed positive staining, the positive staining rate in well differentiated cancer (13of 16,81.3%) was higher than that in moderately differentiated cancer (21of40, 52.5%) and that in poorly differentiated cancer (2of5,40%). The staining intensity was correlated with tumor differentiation and Dukes tumor stage. Gastrin positive granules scattered in cytoplasm of cancer cells. 鈥?Protein A - gold complex was found in density-different secretion granules of cancer cells, in intercellular space and on membrane of microvillus. The results suggested that the expression of gastrin in carcinoma of large intestine might serve as a marker indicating good differentiation. Cancer cells release gastrin mainly with secretion granules. Serum gastrin levels, which were elevated in the group with tissue staining( + + + ) may reflect the gastrin levels in cancer tissues.
