缺氧缺血性脑病新生儿神经元特异性烯醇化酶含量的变化 被引量:3

摘要 目的探讨缺氧缺血性脑病(HIE)新生儿神经元特异性烯醇化酶(NSE)水平的变化,及其对缺氧缺血性脑损伤早期诊断、损伤程度判断和预后评估的临床意义。方法66例确诊为HIE的患儿,分别在出生后24h(急性期)内及日龄第7天(恢复期)采血样送实验室,检测NSE浓度,并与正常对照组(20例)进行比较。观察HIE病因以及脑损伤程度诸因素对血清NSE水平的影响。结果胎儿宫内窘迫组和出生窒息组急性期和恢复期NSE浓度均较正常对照组明显升高,差异有显著性(均P<0.05)。而宫内窘迫组和出生窒息组之间两次NSE浓度比较差异均无显著性。轻度和中、重度组急性期NSE浓度均较正常对照组明显升高,差异有显著性(P<0.05)。轻度组恢复期NSE浓度已接近正常,中、重度组仍高于正常对照组和轻度组,差异有显著性(P<0.05)。结论血清中NSE水平随缺氧缺血性脑损伤严重程度而升高,可作为判断新生儿脑损伤程度的可靠指标。胎儿宫内窘迫所致缺氧缺血性脑损伤与出生后窒息相近,在诊断和治疗上应予以同等重视。
