肺外小细胞癌65例临床观察及分析

Primary extrapulmonary small cell carcinoma: a report of 65 cases

经回顾性调查研究发现，该院１９５８～１９９４年间诊治的６５例肺外小细胞癌中，食管原发小细胞癌最为常见，其首发症状因原发部位不同而各异。５７例有明确预后的患者中，手术组中位生存期１０个月，手术＋化防组中位生存期２３．５个月，局限期患者２年生存率３２．４％，５年生存率８．８％，广泛期患者２年生存率４．３％，５年生存率为０。研究提示：治疗该病除早期切除外，给予系统、足量的化疗将有助于提高患者生存率。

A retrospective study was done on 65 patients with primary extrapulmonary small cell cancer treated between 1958 and 1994 in our hospital. The tumors were located in the nasal cavity, nasopharynx, larynx, thyroid, thymus,esophagus,stomach, cervix, axilla and upper respiratory tract. The initial symptoms were various with the tumor locations. Sixteen patients underwent surgial resection alone with a mean survival of 10 months, while 12 patients were treated by resection in combination with chemotherapy with a longer survival of 23. 5 months. The multimedality therapies in 4 patients had a mean survival of 40. 5 months. The 2-year survival rate of the patients with limited disease is 32.4%. One patient has lived nearly for 10 years after the subtotal esophagectomy. This study suggests that adjuvant chemotherapy would be beneficial to the patients after the surgical operation. And it is even possible to achieve more favorable long-term results by adopting an individualized treatment.
