红外热像图对良恶性乳房肿块鉴别诊断的价值 被引量:2

摘要 对260例单侧性乳房肿块的女性患者作红外热像图检查,按作者提出的五项阳性指标进行判读,其结果与临床诊断和病理检查相对照。恶性病变118例,良性病变142例,用红外热像图作良、恶性病变的鉴别诊断,其敏感性为93.2%,特异性为74.0%,Youden指数为0.67,与病变诊断的符合率为82.7%,大于临床诊断和病理诊断的符合率(64.6%).揭示红外热像图可作为乳腺良、恶性肿块鉴别诊断的有效辅助手段。同时发现对于早期无淋巴结转移的乳腺癌,红外热像图的阳性率也达93.2%,这也揭示红外热像检查可作为乳腺癌筛检手段,用于人群的二级预防,以期提高患者生存率,降低乳腺癌的人群死亡率。 Two hundred sixty female patients with breast lumps including 118 malignant lesions and 142 benign ones were examined with infrared thermography. The thermographic diagnosis was made according to 5 criteria as authors previously reported and the the results were compared with clinical diagnosis and pathological finding. Using pathological finding as gold standard, the sensitivity and the specificity of infrared thermography for differential diagnosis of malignant and benign breast lumps was 93.2% and 74.0 % respectively, and the Youden index was 0. 67. The coincidence rate of thermography with pathology was 82.7% and superior to that of clinical diagnosis with pathology (64.6%). thus infrared thermography may be used as an effective auxiliary procedure for differential diagnosis of breast neoplasia. Results also showed that 93.2% of early breast carcinoma with no metastatic lymph nodes were diagnosed by this technique which suggests that infrared thermography can be applied as a screening measures for early detection of breast cancer.
