冻疮患者运动前后甲襞微循环观察

Observation of Nailfold Microcirculation in Collegians with Chilblain before and after Exercise

本文应用微循环显微电视录像技术及加权积分法观察31例冻疮组大学生及50例健康大学生在运动前后的甲襞微循环变化,探讨冻疮的微循环变化及运动对微循环的影响。结果表明,冻疮患者有轻度的微循环异常(积分值3.71±0.46),显著高于健康对照组(P<0.01),表现为微血管袢顶淤张,血流速度缓慢,袢周有渗出,伴有血管运动性较强及汗腺导管数量增多等交感神经占优势现象。中等强度运动负荷后,冻疮组微循环明显改善,尤以流速改善明显,对照组变化不明显。提示微循环障碍可能与冻疮的发病有关,运动可改善其微循环异常,从而为防治冻疮提供了新线索。

The purpose of this study was to explore the changes of nailfold microcirculation in collegians with chilblain and the influence of exercise on these changes. 31 students with chilblain and 50 healthy students, who had no past history of chilblain were observed. The nailfold microcirculation was visualized and recorded by a vital microscope with TV recorder before and after exercise and analyzed with the multiple quantitative evaluation method proposed by Frof. Tian Niu. The results demonstrated that a light abnormality of microcirculation appeared and the total multiplescores of 3.71±0.46 in chilblain group was significantly higher than that of controlgroup (p<0.0l). The microcirculatory disturbance included: dilated the apices of microvascular loop, slow velocity of blood flow, slow granular flow, and the periv-ascular exudation. At the same time, the sympathetic tone was predominant in microcirculation showing the increased vasomotion and the number of syringadenous catheter. After the exercise with moderate intensity, the microcirculation improved in chilblain group, especially the flow velocity, and the multiple scores were significantly decreased as compared with before exercise (p<0.0l). No signilicant change in microcirculation was found in control group before and after exercise. Results suggest that the patients with chilblain in hand had disturbance of nailfold microcirculation. The pathogenesis of chilblain may have certain relation to the microcirculatory disorder. Exercise might improve the microcirculation of chilblain.
