班秃皮损的组织病理学变化与毛发再生的关系

Observation of the Relation of Histopathological Changes of Alopecia Areata to Pilus Regeneration

为斑秃临床治疗提供形态依据。选择10例男性斑秃皮损标本,取自脱发1天至14年患者头皮,年龄15～44岁。其中活动期3例,稳定期2例,恢复期5例。取4份健康成年男性头皮做对照。石蜡连续切片,分别HE、AB/PAS、网状纤维、弹力纤维、淀粉样蛋白、角蛋白(CK)、S—100蛋白染色观察。皮损内网状支架存在,早期活动期毛囊中下部淋巴细胞浸润显著;稳定期毛母质细胞静止状态,毛囊颈部囊状扩张,内含碎屑样毛渣;恢复期毛母质细胞活化增殖,细小毛发形成渐由毛根长至皮面。应用中药酊剂涂擦皮损可诱使静止期毛球活化,使斑秃毛发得以再生。

To provide a pathologic basis for clinical treatment. Fifteen samples were taken from the scalps of ten male patients,aged 15-44,one day to 14 years after trichomadesis. Their clinical staging were active stage: 3 cases;stationary stage:2 cases and restore stage: 5 cases. The latter was treated with painting of tincture made up by Chinese traditional drugs for 20-30 days. Samples taken from four healthy male youth were also observed as contrast. The samples were incised vertically,paraffin embedding; continues section, stained by HE, AB/PAS,Reticular fibers,Elastic fibers and amyloid. cytokeratin (CK),S-100 for immunohistochemic stain. Network excisted in skin lesion. In the early active stage cases, obvious lymphocytes infiltration was observed arounding the middle and lower parts of folliculus pili. In the stationary stage cases,pilomatricytes were in a resting state,necks of folliculus pili dilatated calabash like,which contained many fragmented pilus fecula. In the restoring stage cases, pilomatricytes activated and proliferated, fine hairs formed in folliculus pili and grew from radix pili to the surface of scaol gradually. Tincture made up by Chinese traditional drugs may induce the resting bulbus pili to activation . Thus, hairs regenerated in the restoring stage of Alopecia Areata.
