心脏瓣膜置换术后心电活动与心肌收缩间隔脱落原因探讨

摘要 目的对体外循环下,心脏瓣膜置换术后并发心电活动与心肌收缩间隔脱落原因及防治进行探讨。方法回顾我院2例风心痛心脏瓣膜替换术前心功能状况,并对术中术后主要环节进行分析。结果2例均于手术后72.小时后心电活动与心肌收缩逐渐恢复一致。于术后15—20天痊愈出院。结论对年青病人,在心肌保护措施上,应针对巨大心肌肥厚伴劳损患者,灵活应用心肌保护方法,一旦发现正常心电活动未激起有效的心肌收缩,应及时用药提高心室率.以防止心室过度膨胀及加重心肌损伤。 Objective To discuss the cause of intervallic hypodynamia in myocardial contraction and the electrecardial circulation. Methods Retrospective review was given to the cardiac function of 2 cases with rheumatic heart disease before cardiac valve replacement;and the main operation procedure was analyzed. Results The accordance of electrocardial action and myocardial contraction was gradually resumed 72 hours after operation,and the 2 patients left hospital 15 and 20 days afterwards. Conclusions For young patients, especially those have ventricular hypertrophy with strain, cardiac muscle protection measures should be given;one normal electrocardial action but with intervallic hypodynamia in myocardial contraction is detected, drugs should be given timely to raise the ventricular rate so as to prevent the over-swelling of the ventricle and severe myocardial damage.
