食管癌放疗加光动力学疗法随机研究结果

RADIOTHERAPY PLUS PHOTODYNAMICS IN TREATING ESOPHAGEAL CANCER FINDINGS OF A RANDOM STUDY

１９９１年１０月至１９９２年１２月，作者在前期研究的基础上，采用食管癌放疗加光动力学疗法６０例随机分组研究。对照组：３０例，单纯体外放疗ＤＴ６０～６６Ｇｙ／６～７周；实验组：３０例，体外放疗同对照组，只是当Ｄｒ４０Ｇｙ／４周时加用ＰＤＴ。结果显示：对照组和实验组近期疗效（Ⅰ＋Ⅱ级Ｘ线片）、１、３年生存率对照组分别为：６６．７％（２０／３０）、４６．７％（１４／３０）、１６．６％（５／３０）；实验组分别为：８３．３％（２５／３０）、７６．７％（２３／３０）、３０．０％（９／３０）。实验组高于对照组，其中１年生存率实验组明显高于对照组，（Ｐ＜０．０５）。作者认为放疗加光动力学疗法优于单纯放疗，是治疗中晚期食管癌的一种有效方法。

rom October l99l to December l992,on the basis of the firststage(prophase)researchwork,radiotherapy combined with photodynamics were adopted for 60 cases of esophagealcancer,which were randomly divided into two groups the control group(CG)and the treat-ment group(TG),each group comprising 30 patients,For the CG,the approach of DT6066Gy for 6～7 weeks for extracorporeal radiation was adopted.Extracorporeal radiation giv-en to the TG was the same as that offered to the CG,except for the addition of PDT whenthe DT 40Gy was given for 4 weeks,The short-term curative effect,one-year and three-yearsurvival rates seen in the CG were 66.7%(20/30),46.7%(14/30)and l6.6%(5/30)respec-tively,Such rates happening in the TG were 83.3%(25/30),76.7%(23/30)and 30,3%(9/30) respectively.What was shown in the TG was apparently higher than that seen in the CG(P<0.05).The author holds that radiotherapy combined with photodynamic therapy wassuperior to using radiotherapy alone,representing an efficient way in the treatment of advanced carcinoma of the esophagus.
