摘要
目的提高对老年冠心病患者并发肠缺血的认识,完善冠心病防治规则。方法选择经临床和影像检查确诊的36例老年(65。74岁)冠心病患者与31例中青年(35—50岁)冠心病患者的肠系膜上动脉造影影像进行对比分析,分析年龄与肠系膜上动脉之间以及冠状动脉病变程度与肠系膜上动脉病变之间的关系。结果观察组36例中19例(52.8%)肠系膜上动脉病变,对照组31例中3例(9.7%)肠系膜上动脉病变(P〈0.05)。观察组冠状动脉多支血管病变者24例(66.7%),对照组多支血管病变者7例(22.6%)(P〈0.05)。冠状动脉多支血管病变者31例(46.2%),合并肠系膜上动脉病变19例(19/22,86.3%);单支血管病变36例(53.7%),合并肠系膜上动脉病变3例(3/22,13.6%)(P〈0.05)。结论冠心病患者的年龄与肠系膜上动脉病变之间有紧密相关性,年龄越大,冠状动脉损害越严重,肠系膜上动脉损害多在冠状动脉损害较严重时发生.应重视老年冠心病患者并发肠缺血的存在。
Objective To make further cognition of coronary artery disease (CAD) companied with mesenteric ischemia and perlect the layout of prevention and cure of CAD patients. Methods Contrast 36 elderly patients (65-74) with CAD proved by clinic or radiological examination with 31 young patients (35-50) with 31 young patients (35-50) with CAD,analyzing the superior mesenteric angiographic images and the associativity of ages and images, also researching associativity of the degree of pathological changes between t.oronary artery and superior mesenterie artery. Results There are 19 ( 52. 8% ) patients accompany with superior mesenteric arterial lesions in observe groups,but there are 3(9.7% ) cases in control groups (P 〈 0.05), and there are 24 ( 66. 7% ) patients belong to multivessel disease in observe groups,and there are 7(22. 6% ) patients in control groups (P 〈 0. 05). There are 19(19/22,86. 3% ) patients accompany with superior mesenteric arterial lesions in 31 patients with multivessel disease, and there are only 3 (3/22, 13.6% ) patients in 36 patients with alone vessel disease ( P 〈 0.05 ). Conclusion The ages are strongly associated with superior mesenteric arterial lesions in CAD patients, the older the patients are, the severer the damage of coronary artery and the damage of superior mesenteric artery is based on severe damage of coronary artery. So we should think highly of ischemia of intestine in elderly CAD palients.
