新生儿缺氧缺血性脑损伤血胃动素和促胃液素水平的相关性研究

Related study on blood motilin and gastrin levels in neonate with HIBD.

目的探讨新生儿缺氧缺血性脑损伤(HIBD)对胃肠激素分泌的影响及其相关性的意义。方法应用放射免疫分析法(RIA)测定67例新生儿窒息患儿(包括45例HIBD患儿和22例非HIBD患儿),病程2d内,3d^5d,7d^10d,12d^15d血浆胃动素(MTL)和血清促胃液素(GAS)的含量。据临床表现,将HIBD组分为3个观察组,将血MTL和GAS水平的动态变化与其阴性对照组比较,并对其数据进行统计学处理。结果1HIBD患儿在整个病程中,血MTL和GAS升高明显(P<0.05)。2非HIBD新生儿血MTL和GAS与对照组比较,差异无显著意义(P>0.05)。3伴有惊厥、胃肠功能降低、持续的胃肠喂养困难,血MTL和GAS水平升高明显,并较其他组下降缓慢。结论HIBD患儿存在血MTL和GAS分泌异常,血MTL和GAS水平与脑损伤的程度、胃肠功能及病情的恢复密切相关。

Objective To explore the effect on gastrointestinal hormone secretion caused by neonatal HIBD and its significance of correlation. Methods Plasma motilin and serum gastrin concentration were measured by radioimmmunoassay in 67 newborns with asphyxia （including 45 newborns with HIBD and 22 newborns without HIBD） in the 0～2, 3～5, 7～10, and 12～15 days of the course of disease. According to the clinical manifestations, we divided the asphyxia group into three groups, and compared with the negative control group, then we did statistical analysis. Results ① The blood MTL and GAS concentration of neonatal HIBD increased dramatically during the whole course of the disease （P〈0. 05）. ② The blood MTL and GAS concentration of newborns with no HIBD has no statistical significance compared with the normal control group （P〉0.05）. ③The blood level of MTL and GAS increased obviously in the groups which are complicated by convulsion, gastrointestinal dysfunction, persistent difficulty in feeding, and decrease more slowly than other, groups. Conclusion The HIBD of newborn can cause abnormal secretion of plasma motilin and serum gastrin. The blood level of MTL and GAS has close relationship with the degree of brain injury,gastrointestinal function and the recovery of the disease.
