彩超引导下介入治疗在肝外伤中的应用价值

摘要 目的:研究彩超引导下介入治疗对肝外伤患者血肿形成后难以吸收者的应用价值,旨在提高超声诊断及治疗水平。方法:分析37例接受非手术治疗(nonoperation management,NOM)的肝脏外伤患者的声像图特征,将其中24例肝脏血肿面积超过5cm×5cm的患者随机分为两组,介入治疗组14例,非介入治疗组10例,观察比较两组血肿面积缩小50%所需时间及血肿完全或近乎完全消失所需时间。结果:介入治疗组血肿缩小50%所需时间及血肿完全或近乎完全消失所需时间明显短于非介入治疗组(P<0.01)。结论:对血肿面积超过5cm×5cm的NOM肝脏外伤患者行介入治疗可以显著地缩短血肿消失时间。 Objective:To study the applicated value of interventional treatment guided by color doppler ultrasonography in hepatic trauma with nonabsorbing hematoma. Methods:The ultrasound features of 37 cases who received nonoperation management (NOM) were statistically analyzed. 24 cases with hepatic hematoma area exceeded 5 × 5cm^2 were randomly divided into interventional treatment group (n = 14)and non- interventional treatment group( n = 10) ,and their average time taken for 50%reduction and complete or nearly complete disapperance of the hematoma cavity were recorded and compared. Results:The average time taken for 50 96 reduction and completely or nearly completely disapperance of the hematoma cavity in interentional treatment group was significantly shorter than that in non--interventional treatment group ( P ( 0.01 ) . Conclusion: Interventional treatment guided by color doppler ultrasonography guidance may shorten the average time taken for disappearance of hematoma.
