吉西他滨联合卡培他滨治疗铂类方案失败的晚期鼻咽癌的临床研究

Clinical observation of gemcitabine combined with capecitabine for advanced nasopharyngeal carcinoma patients after failure of platinum-based chemotherapy

目的:观察吉西他滨联合卡培他滨治疗铂类方案失败的晚期鼻咽癌的临床疗效和毒副反应。方法:吉西他滨1.0g/m2,分别于第1天和第8天静滴,卡培他滨1000mg/m2,2次/日,第1天至第14天,21天为一周期,所有病例均接受至少2个周期的化疗。结果:23例患者入组,均可行疗效及毒性评价,其中完全缓解2例,部分缓解8例,总有效率43.5%。主要毒副反应为骨髓毒性及手足综合症,均较轻。结论:吉西他滨联合卡培他滨方案治疗铂类方案失败的晚期鼻咽癌有较好的疗效,毒副反应小,值得临床进一步研究。

Objective：To investigate the efficacy and toxicity of gemcitabine combined with capecitabine in recurrent or metastatic nasopharyngeal carcinoma （NPC） after failure of platinum - based chemotherapy. Methods： Gemcitabine 1000 mg/m^2 infusion on days 1 and 8, and oral capecitabine 1000 mg/m^2 twice daily for 14 days were administered every three weeks. All patients received at least two cycles of the treatment. Results： Twenty - three patients were enrolled. All patients were included in the efficacy and adverse events analysis. There were two complete responses,eight partial responses, overall response rate was 43.5%. The major toxicity included myelotoxicity and hand- foot syndrome were mild. Conclusion： Gemcitabine combined with capcitabine demonstrates clinical activity and an acceptable safety profile in patients with recurrent or metastatic NPC who had experienced treatment failure with platinum -based therapy. Further investigation is warranted.
