2型糖尿病尿微量白蛋白与炎症介质的相关性 被引量:2

Relationship Between Microalbuminuria and Inflammation Media in Type 2 Diabetes Mellitus

摘要 目的研究T2DM尿微量白蛋白水平与炎症介质C-反应蛋白(CRP)和白介素-6(IL-6)之间的关系,并探讨其临床意义。方法96例T2DM患者根据24 h尿微量白蛋白排泄率(UAER)分为:无蛋白尿(NA)、微量白蛋白尿(MA)和临床蛋白尿(CP)3组,检测患者的CRP、IL-6、血糖、血肌酐和胰岛素。结果MA组CR和IL-6水平显著高于NA组(P<0.05,P<0.01),CP组CRP、IL-6、病程、肌酐、胰岛素抵抗指数亦显著高MA组(P<0.05,P<0.01)。CRP、IL-6水平与UAER、稳态模型胰岛素抵抗指数(HOMA-IR)、体质量指数(BM I)呈正相关(P<0.01)。结论T2DM CRP和IL-6炎症因子与胰岛素抵抗密切相关,是反映其肾脏受损程度的标志之一。 Objective To study the relationship between microalbuminuria and inflammation media of C-reactive protein (CRP) and intedeukin-6 (IL-6) in type 2 diabetes mellitus, and to explore the clinical significance. Methods Ninety-six patients with type 2 diabetes mellitus were divided into 3 groups according to the urine albumin excretion rate (UAER) : normal albuminuria group (NA) , microalbuminuria group (MA) and clinical proteinuria group (CP). The serum levels of CRP, IL-6, fasting plasma glucose, creatinine and fasting insulin were measured in all the cases. Results The levels of CRP and IL-6 in MA group were significantly higher than those in NA group (P 〈 0.05, P 〈0.01 ), and the data of CRP, IL-6, disease duration, creatinine and insulin resistance index in CP group were also higher or longer than those in MA group (P 〈 0.05, P 〈 0.01). CRP and IL-6 were positively correlated with UAER, homeostasis model assessment of insulin resistance, and body mass index (P 〈 0. 01 ). Conclusion CRP and IL-6 are closely related to insulin resistance in type 2 diabetes mellitus, and they are among the renal injury signs.
