腹腔镜脾切除术100例

Laparoscopic splenectomy in 100 consecutive cases

目的总结8年多来行腹腔镜脾切除术(LS)的临床经验,探讨LS的安全性和有效性。方法从1996年4月至2004年11月连续进行了100例LS,94例为原发性和继发性脾功能亢进,6例为其它脾疾病,其中34例为巨脾。结果100例LS全部完成,1例术后5h需作小切口开腹止血,2例术后需再次腹腔镜探查和清除脾窝积血。平均手术时间2.3 h,平均失血160 ml。平均术后住院6 d。结论LS安全、有效、切实可行,适用于有脾切除指征的原发性和继发性脾功能亢进或其它脾疾病。

Objective To summarize the experience in laparoscopic splenectomy （LS） for 100 consecutive patients and to explore the safety and efficacy of LS. Methods A total of 100 patients underwent LS from April 1996 to November 2004 in hospital. Of the 100 patients, 94 suffered from primary or secondary hypersplenism and 6 from other diseases. Thirty-four patients had massive splenomegaly. Results LS was performed in all the 100 patients and there was no death. One patient received a small laparotomy to stop bleeding 5 h after LS and 2 underwent the second laparoscopic procedure for clearing of clots. The average operative duration was 2.3 h, mean blood loss 160 ml and median postoperative duration of hospitalization 6 d. Conclusions LS is a feasible, effective and safe surgical procedure for patients with indications of splenectomy.
