两种化学氧化法测定血清胆红素的比较

摘要 目的:比较钒酸盐氧化法和化学氧化法测定血清胆红素的性能。方法:用钒酸盐氧化法、化学氧化法分别测定血清中总胆红素(TBil)和直接胆红素(DBil),比较两种方法的精密度、线性范围、回收率和抗干扰能力,并计算两种方法测定结果的相关性。结果:两种方法重复性实验批内变异和批间变异分别小于3.0%和5.0%;两种方法测定TBil的平均回收率分别为100.2%、101.1%;测定DBil的平均回收率分别为99.4%、98.4%。钒酸盐法测定TBil的线性范围为0~570μmol/L,化学氧化法测定TBil的线性范围为0~650μmol/L,钒酸盐法测定DBil的线性范围为0~312μmol/L,化学氧化法测定DBil的线性范围为0~213μmol/L。以钒酸盐方法为对比法(Y),对化学氧化法(X)进行评估,两种方法测定TBil回归方程为Y=0.995X+1.954,相关系数r2=0.999,(P>0.05),测定DBil的回归方程为Y=0.940X+0.731,r2=0.996,(P>0.05)。化学法抗溶血干扰能力较钒酸盐法强,而抗维生素C的干扰较钒酸盐法差,两种方法对TG的抗干扰能力相当。结论:钒酸盐氧化法和化学氧化法测定血清胆红素性能相当,两种方法的线性检测范围、精密度、回收率和抗干扰能力均符合临床常规使用要求。 Objective:To compare the performances of the vanadate-acid oxidize and chemical oxidize methodsto detect bilirubin in serum.Methods:The total bilirubin and direct bilirubin in serum were detected by the methods of the vanadate-acid oxidize and chemical oxidize,respectively.The precision,linearity,rate of recovery,anti-interference ability of two methods were compared according to the standard method of NCCLS,and the relativity of two methods was calculated.Results:The within-run and run to run imprecisions (CV) of two kinds of methods were less than 3.0% and 5.0%,respectively.The recovery rate of two kinds of methods for detecing total bilirubin was 100.17%, 101.12%, respectively, and detecting direct bilirubin was 99.36%. 98.35%, respectively.The linear range of TBil measured by the method of the vanadate-acid oxidize was 0-570 μmol/L and that by the chemical oxidize was 0-650 μLmol/L, the linear range of DBil measured by the method of the vanadate-acid oxidizes was 0-321 μmol/L and that by the method of the chemical oxidize was 0-213 μmol/L,respectively.The TBil levels by vanadate-acid oxidize method (X) were closely correlated with those obtained by chemical oxidize method (Y),Y=0.995X+1.954,r^2=0.999 (P〉0.05),the DBil levels by vanadate-acid oxidize method (X) were closely correlated with those obtained by chemical oxidizes method (Y), Y=0.940X+0.731, r^2=0.996, (P〉0.05).The interference of the anti-dissolving blood in the chemical oxidize method was stronger than that in the vanadate-acid oxidize, but the interference of the anti- vitamin C was less than the vanadate-acid oxidize method.The two kinds of methods were equal to the interference ability of anti-TG.Conclusion:Examination range, anti-interference ability, precision and recovery rate of bilirubin determination with the two methods can meet the clinical demand,suitable for determination of bilirubin levels in a clinical laboratory.
