经尿道膀胱肿瘤电切加术后膀胱灌注112例疗效分析

Clinical effects of transurethral resection of bladder tumor and pastoperative irrigation of bladder:A report of 112 cases

目的:探讨经尿道膀胱肿瘤电切术(TURBt)加术后膀胱灌注治疗膀胱肿瘤的疗效。方法:对112例膀胱肿瘤患者行TURBt加术后膀胱灌注丝裂霉素治疗,其中21例合并良性前列腺增生症(BPH)者同期行经尿道前列腺电切术(TURP)。结果:112例患者成功实施TURBt142台次。均获得随访6个月～9年,肿瘤复发27例,2例改行全膀胱切除术,22例再次行TURBt,3例死于广泛转移。结论:TURBt加术后膀胱灌注是治疗表浅性膀胱肿瘤的有效方法,具有安全、可靠、损伤小、术后恢复快等优点。

Objective：To investigate the selection of indications and surgical techniques of transurethral resection of bladder tumor （TURBT） and the chnical effects of TURBt and postoperative irrigation of bladder in the treatment of bladder tumors.Methods：TURBT plus postoperative intravesical irrigation of mitomycin c was performed in 112 patients with bladder tumor,including male 84 and female 28.There were 38 cases coses coexisting with benign prostatic hyperplasia （BPH）,21 of them underwent transurethral resection of prostate （TURP） simultaneously.The clinical data were analyzed retrespectively.Resuts：All the 112 cases underwent TURBt successfuUy in 142 times,without serious intraoperative and delayed complications.After 6 months to 9 years follow up,tumor recurrence happened in 22 cases.Among them,22 cases were performed TURBt once again,2 cases were changed to undergo radical cystectomy,3 cases died of bread cancer metastasis.Conclusion：TURBt combined with postoperative intravesical irrigation of mitomycin c in safe and weliable carative method,with the advantages of less lesion,faster recovery and so on.
