原发性巨大骶骨肿瘤的手术治疗

Surgical treatment for large primary sacral tumors

目的探讨骶骨肿瘤手术治疗的方法。方法分析5例巨大骶骨肿瘤患者的手术治疗方式、术中出血、骶神经保护和骶骨重建等问题。结果5例患者通过适当的手术方法均得到肿瘤全切,术中出血量较少,大小便功能得到有效保护,术后骶骨-骨盆稳定性良好。结论恰当的手术方式、术前或术中行栓塞治疗、术中注意保护骶神经有利于提高骶骨肿瘤的治疗效果。全骶骨切除术后腰椎和骨盆内固定术有利于保持腰椎及骨盆的稳定性。

Objective To investigate the strategies of surgical treatment for sacral tumors. Methods Five cases of sacral tumors were studied in regards to their surgical＇methods, bleeding treatment, sacral nerve protection and reconstruction of sacrum. Results Through proper surgical methods, 5 cases of sacral tumors achieved total resection, less bleeding, effective preservation for function of urination and defecation and good stability of sacrum after operation. Conclusion Proper surgical methods, preoperative or post-operative embolotherapy and attention to protecting the function of sacral nerve are all benefit to improve therapeutic outcomes of sacral tumors. Internal fixation of lumbar and pelvis after the total resection of sacral tumors are effective means of achiving the stability of sacrum.
