外伤大骨瓣一期分瓣回置的临床应用研究

Clinical application of large bone flap-separating replacement at traumatic craniotomy

目的探讨外伤大骨瓣一期分瓣回置的临床可行性。方法研究组40例标准外伤大骨瓣一期分瓣回置,对照组45例行大骨瓣去除减压,统计比较两组术后颅内压、GOS、并发症情况。结果两组病人颅内压、GOS比较差异无显著性意义,研究组并发症明显减少,回置骨瓣能够骨性愈合。结论大骨瓣一期分瓣回置既能有效降低颅内压,又能减少并发症。

Objective To research the clinical feasibility of replacing the bone flap with several separated segments in the standard large traumatic craniotomy. Methods In all 85 cases undergoing standard large traumatic craniotomy, the bone flaps of 40 cases as study group were replaced by separated segments, and the bone flaps of 45 cases as controls were removed at craniotomy. Their intracranial pressure, GOS and complication were compared statistically by statistics test. Results The intracranial pressure and GOS between the two groups were not significantly different. The replaced bone flap developed into synostosis, thus preventing complications markedly. Conclusion Replacement of the bong flap with several separated segments in the standard large traumatic craniotomy may not only lower the intracranial pressure efficiently, but also reduce complications.
