咽旁间隙肿瘤的诊断及手术入路

Diagnosis and surgical approach for parapharyngeal space neoplasms

目的探讨对咽旁间隙肿瘤的诊断与手术人路的选择。方法对1984～2004年82例咽旁间隙肿瘤的临床资料进行回顾性分析。结果82例中良、恶性肿瘤分别占87.80％、12.20％。以神经源性(50％)及涎腺肿瘤(26.83％)最常见。手术采用口内径路8例,颈侧径路62例,颈颌径路9例,颈腮腺径路3例。结论颈部彩超及CT是诊断、鉴别诊断的良好手段,颈侧径路则是最常用而且安全的手术径路方式。

OBJECTIVE To explore the diagnosis and surgical approach for parapharyngeal space neoplasms. METHODS 96 cases of patients with parapharyngeal space neoplasms from 1984 to 2004 were analysed retrospectively. RESULTS Benign and malignant tumors account for 87.80 % and 12.20 % respectively. The most common types were neurogenic（50 % 41/82） and parotid（26.83 % 22/82）. The trans-oral approach was used in 8 cases, the trans-cervical was used in 61 cases, the trans-cervical and jawl in 9 cases, the trans- parotid in 4 cases. CONCLUSION Ultrasound and CT is a good method for diagnosis of parapharyngeal space neoplasms, The trans-cervical approach was used most often and the most safe approach.
