33例胃癌术前行HSAPC的疗效分析

作者对33例经病理组织学确诊的胃癌患者,于术前行高选择性动脉插管化疗(HSAPC),其中27例进行了手术治疗。与同期术前全身静脉化疗的10例胃癌患者比较,采用本法治疗除临床症状有不同程度的改善外,术后病理检查显示原发灶和淋巴结中的癌细胞均有不同程度的变性坏死,其近期有效率为95.65%。对照组有效率为20%,两组比较差异显著(P<0.05)。作者认为,胃癌患者术前行选择性化疗近期抗癌效果好,毒副反应轻,不仅能提高手术切除率和根治率,而且对减少或防止术中癌细胞的医源性扩散、种植及术后复发,均有重要的临床意义。

Abstract The effect of high selective arterial perfusion chemotherapy(HSAPC) in 33 cases of gastric cancer treated has been studied, 27 of which underwent Following operation,and in comparison of 10 cases gastric cancer treated with the preoperative vein chemotherapy(control group) The removed specimen were studied microscopically to show the degeneration and necrosis of the carcinous cells. After the treatment of the carcinous cells,all the 27 cases in study growp showed various degrees of degeneration and necrosis,Effective rate was 95. 65%. On the other hand ,the contral group was only 20%(P<0. 05). We conclude that preoperative by HSAPC has good short-term effect and little side-effect and could effecctively increase the rate of surgical removement ang redical resection rate, and prevent the intraoperative iatrogenic metastases and implantation,and decrease the postoperative recurrence of the patients with gastric cancer.
