摘要
作者分析了35例位于粘膜和粘膜下层的早期胃癌。97%早期胃癌存在不同程度的临床症状。其中14.3%周围淋巴结已有转移,且粘膜下癌转移率为33.3%。作者认为,位于粘膜和粘膜下层的胃癌,只有无周围淋巴结转移者,才能称为早期胃癌。故对粘膜和粘膜下胃癌需行包括周围淋巴结扩清的胃癌根治术。并认为对这种癌只做内窥镜等局部切除手术应慎重为宜。
Abstract This retrospective study is concerned with 35 patients with early gastric carcinoma located in mucosa and submucosa. About 97 percent of early stage carcinoma showed either prolonged history of gastric abnormality and sudden deterioration ,or epigastric pain with healthy physical condition. All are highly suspected to be malignant and need further inspection. Of the 35 patients , 14. 3% had lymphatic metastasis and the metastasis ratio of submucosa carcinoma was 33. 3% ; We consider the mucosa and submucosa carcinoma without lymphatic metastasis as early gastric carcinoma. The radical curation with lymphatic resection is indicated for mucosa and submucosa carcinoma.Local enuleation such as resection by fibroendoscopy shoutd not be recommended.
